Admission Requirements

All necessary enrollment forms must be completed and returned prior to admission.  It is the parent’s responsibility to ensure all forms are updated immediately upon changes and at least once a year. 

Child's immunization must be up to date before starting the daycare.

Child Care Fees

Child care fees are due biweekly by cash/check. If payment is not received by the due date a late fee of $25 will be charged.  

 

Multiple late fees could result in immediate termination of your contract and you will lose your position.  

 

Returned checks will incur an additional fee of $25 along with the late fee.  

Fees for days your child is absent will still apply.

Late Fee Charges

There is an hourly $10 charge for early drop-off before 8am. A late fee of $2 per minute will be applied for after 6pm pickup. The fee is due at the time of pickup.

Withdrawal Policy

Parents will need to provide one month written notice when decided to withdraw their child from the daycare. 

The Daycare Provider can terminate the contract if the parent does not abide by the contract rules and policies or if the child is not a good fit for the daycare.

Vacation Policy

Parents will need to provide 2 weeks notice of any planned vacation. Families are charged full tuition for vacation week. The fee should be paid in advance.  

Required Reporting

The family child care provider is a mandated reporter of suspected abuse, neglect, or deprivation of a child. This means the law requires the provider to report any known or suspected abuse, neglect, or deprivation to the Department of Family and Children Services.  

Open Door Policy

Parents may visit the family child care learning home unannounced and at any time that their child is in care. Any information requested by the parent concerning the operation of the family child care learning home or the care of the child will be provided to parents.

The parent(s) will be provided daily communication (verbal/written) regarding the care of the child, especially with infants, toddlers, and nonverbal children.

Infant Sleep Position Practices

Based on the risk factors of Sudden Infant Death Syndrome (SIDS) all infants will be placed to sleep on the infant’s back unless the home has been provided a physician’s written statement authorizing another sleep position for that particular infant.   

All infants will be placed to sleep on a firm, tight-fitting mattress in a sturdy and safe crib with no pillows, quilts, blankets, comforters, bumper pads, sheepskins, stuffed toys, or other soft items in the crib. Form 9227 should be fill out for each infant.

 

The infant’s sleeping area will be maintained within a temperature range of sixty-five 65° to eighty-five 85° degrees depending upon the season.  

Food Allergies Form

Food Allergies form must be completed, signed and returned to the daycare provider.   It is the responsibility of the parents to notify the daycare provider of any changes to the list.  

Communicable Diseases

For protection of your child and other children, a sick child can not be admitted to daycare until a doctor's note is provided saying he/she is completely well.  Below are some examples of common diseases:

  • Fever of 100 degree Fahrenheit orally

  • Cold with yellow or green nasal discharge

  • Constant clear nasal discharge

  • Flu

  • Strep Throat or other streptococcal infection

  • Stomach Flue

  • Persistent diarrhea (including reaction to antibiotic)

  • Productive or persistent cough

  • Vomiting

  • Ringworm

  • Chicken Pox

  • Mumps

  • Impetigo

  • Scabies

  • Head Lice

  • Rubella

  • Pertussis

  • Measles

  • Rash
